HEALTH INSURANCE
- Ask the client if they already have insurance, and if so, which insurance company/plan they have. Specifically, ask clients if they have Medicaid or Medicare Some clients will not know which one they have; you can ask them to send a picture of their card. Medicare is funded federally, Medicaid is funded state + federally. If you see a New York state icon on their card, there is a good chance your client has Medicaid. Medicare vs. Medicaid
- If your client is uninsured and lives in New York, we can get them set up with our REAP office to get Medicaid.

Sample script:
- “Hi Suzy, this is _____ from the ACCESS team at MSHRP. One of our clients (Case #: XXX, initials: XXX) needs health insurance; would you be willing to help us get the client set up with Medicaid? Please let us know what the next steps are” Include the case number in all correspondence. Suzy is a rockstar and will make it happen. Follow up with your client to ensure they were able to apply and to please let you know when they hear back.
